A fundamental step for Italian territorial assistance has been taken with the signing of the agreement that regulates the work of general practitioners in Community housesThe agreement, reached between Sisac which represents the Italian Regions and the unions Fimmg e fmt introduces new rules to ensure a more efficient and sustainable service.
The agreement, signed in the evening, establishes that general practitioners will have to dedicate up to 6 hours per week for 48 weeks per year in the Community Houses, with shifts of at least 3 hours continuous between 8pm and 00pm.
For each hour of activity, doctors will receive a compensation of 38,72 € in addition to charges, with a single tariff throughout the national territory.
The innovations introduced by the agreement
The agreement provides that each Health company define the hourly requirements of the facility, after having employed the personnel already assigned to hourly activities and having consulted the contact person of theFunctional Territorial Aggregation (Aft).
The remaining hours will be distributed among the doctors working in the territory of the Community House.
The Minister of Health, Horace Schillaci He had initially proposed an ad hoc decree to fill the new facilities with an adequate number of doctors, but the idea was shelved following controversy and opposition from doctors' unions.
The agreement reached unblocks the situation, providing a national direction and preventing each region from proceeding independently.
The position of the unions

On the opposite side, the trade unions Come on e snami They announced that they will not sign the agreement, denouncing a "distortion of the legal nature of the employment relationship" of general practitioners.
The next steps
The agreement reached will now have to follow its procedural path to enter into force by June 30, 2026 in compliance with the deadlines set by the PnrrThe agreement also provides for the possibility that hospital doctors may work in community homes on a voluntary basis and outside of working hours, eliminating some incompatibilities.
